I think you should call St. Lukes to set an appointment as walk in may be refused as I am sure they are busy.

Hello! Alexandria! Oo tumatanggap sila kasi ganon din ginawa ko Upfront Medical. Kakamedical ko lang nung April. Basta sabihin lng ng husband mo for Upfront medical, alam n nila yun. Sa St. Lukes Global City ako ngpamedical.

Anyone here experience submitted an outdated form? For example, IMM0008 I downloaded the Feb 2015 version, then CIC updated the form to APril 2015. But you submitted your application June 2015. I checked the difference between the forms, wala naman sya pinagkaiba sa hiningi na info they just added some disclosures sa last page. Ano po magiging possible problem if ganito po nangyari? TIA

Meron ba didto nag expire ang date of initial entry because hindi dumating ang passport/visa. Im worried my husband's initial entry will expire Sept 17, 2015. CEM received passport May 8. DM June 4. Wala pa rin ang passport niya.

Hi everyone, I just want to ask if anyone of you had experience this and hope it will give me inputs for I am so stress right now.

I just got my AOM "Advisory on Marriage" at NSO but upon checking the Spelling of my Husband's name is incorrect from "KRISTOFFER" to "KRISTOPFER". We already amended our marriage certificate before since there were also some typo errors, confidently knowing that the spelling of my husband's name on the upper portion of the form is correct but upon checking it was wrong and NSO told me that it was the spelling entered in their system.

What should I do?

Please I need every inputs and we'll be appreciated.

I need your help po.. sana matulungan nyo ako. plan ko po sponsoran ang hubby ko dito sa canada. umuwi po ako para magpakasal. civil wedding kmi. ung ceremony at reception ginanap sa restaurant sa makati pero nung makuha na namin ang Marriage Certificate from NSO, ang nakalagay na Place of Marriage is sa City hall ng MAnila. dun din sa manila city hall ni register ang kasal. dun din po kmi kumuha ng license sa manila city hall. may nabasa po ako sa forms asking where and when the marriage took place and who were the attendees. lahat ng photos namin nung kasal sa restaurant po lahat. magkakaproblema po ba kami? and ung kasal po pla namin wala po kaming mga parents na umattend. mga kapatid lng namin at pinsan hindi namin pinaalam sa magulang namin kc gsto nila sa church kami ikasal active kc sila sa simbahan kaya di namin pinaalam..gusto ko lng maimulan na agad ung pag sponsor saknya pra mapaghandaan namin ang church wedding. ngayon stress po kmi pag prepare ng application namin. matagal na po kmi magkarelasyon 5 years na. salamat po sa sasagot.

Hi. I saw your post about amendment of you Marriage Certificate. what was the problem and how did u have it fixed? Our marriage certificate needs to be amended too because of the wrong place of marriage. please reply.. thank you very much.

Hi Sis, ako din ung nag post ng amendment sa Marriage cert ko na ban kasi ako dito last time kaya dinelete ko ung account then created anotehr account and only to find out because of IP lang pla kya nka ban ako but anyway, regarding your concern i think okay naman na Manila ung nakalagay dun sa place of event ninyo pero ung sa akin kasi ung events place tlga ng wedding which is "Light of Love" sa Quezon City.

Well, ung pag ammend nung saken kasi due to typo/clerical error so pumunta ako Quezon City hall then nag punta ko local civil registry nila then as k for requirements na need ko for petition so first petition ko dami nila hiningi like (TOR sa school, baptismal, etc,.)Depende ata sa City hall yan tapos nagbayad ako ng 1,500 pero since pina rush ko ung saken so nagpa additional sila ng 5000 pero walang receipt na binigay saken jan and I dont know why pero ung 1,500 meron OR. Tapos mga 2 weeks lang na amend na at nag appear na sa NSO ung amendment ng errors. but upon claiming ung AOM ko this month lang sa NSO Makati may error na naman ung name ng husband ko kasi ung daw ung pagkabasa nila sa system at sa sinabmit na papers sa knila though sa LCR tama naman pero sa system nila mali i need to ask again another petition (haaizt ) so ayun magpapasa ako ngaun ng same docs na lang from my previous application for petition sa LCR ng QC pra mag amend ulit and this time ndi ko na pina rush 2 months ung process kasi ang gagawin ko magpasa na lang ako Sworn Affidavit tapos ung petition letter na galing ng LCR then once ma transfer ung papers ko dito sa CEM and mag ask sila new Marriage cert at AOM kasama ng PPR then meron na ako amend copy.

Nakalagay sa marriage Cert namen Events place ng wedding namen which is "Light of Love Events place (Atrium) and then the address. But since part yan ng QC, Sa QC sinubmit ng pastor namen ung papers namen to register.

Mga names din namen sis. Nagkamali kasi ung pastor namen na sinubmit nya agad ng hindi nya napapa check sa amin eh un ang dami pla mali sa mga names namen. Haizt.. Pero okay lang nkaka move on naman na kahit papano. Tom, papadala ko na sa sponsor ko ung mga papers kahit ndi pa ako nkakapag second amend nagpagawa na lang ako affidavit of discrepancy to explain then saka ko ipapa amend ulit kc 2 months un to process eh. if ma transfer na ung papers ko dito sa manila at humingi sila new copy may mabibigay na ako for sure na new marriage cert

Sis, i asked ung friend ko na nasa Canada na ngaun last june 22, 2015 lang sya dumating kasi kinasal un sa Cabalen Restaurant eh sa Market Market Taguig, sabi nya un daw nakalagay sa marriage cert nya "name ng resto/place then the address" Kung san daw kasi nagka pirmahan. So ayun. sana nakatulong ako sau..
