Good morning,

I will be sponsoring my husband. I renewed my Philippine passport on my married name while my PR card still has my maiden's name. Also, last week I updated my OHIP with my married name.

*In the Philippines, we usually retain our maiden's last name as married middle name while here in Canada middle name stays the same.

Maiden's Given Name: A
Maiden's Middle Name: B
Maiden's Last Name: C
Husband's Last name: D

PH passport: ACD
PR card: ABC, still in my maiden name
OHIP: ABD

What name should I put in my application? Should I go with what's in PR card?
I actually called Immigration to ask, they told me to go with what's in my PR card and just provide a letter of explanation that my PR still has my maiden's name as I received it when I was still unmarried.

I just want to validate based on your experiences with Immigration.

As a Canadian PR, I believe you are not required to submit a copy of your passport. As a sponsor, you need to submit a copy of your PR card. If you have time, I would suggest requesting a new PR card with your new name (ABD) as on your OHIP. Once you get your new PR card, you can file your application to sponsor your husband.

Hi, thank you for your reply.
Another concern is if I align my PR card name with my OHIP (ABD), it will not match my passport (ACD). When I go out of the country, do you think I might run into issues with Immigration?

@eolofernes your PR card (ABC) currently does not match your Philippine passport (ACD) anyways, so yes you can potentially face some problems with immigration if you are travelling. However, IRCC will still have your UCI which is unique for you, and they will know your previous passport name (ABC), previous PR card name (ABC) and new PR card name (ABD) if you do decide to change your PR card.

If you are planning on travelling, do keep all documentation to prove what happened and you shall be fine. I would recommend to restrict your travel plans as much as possible.

As a Canadian PR, I believe you are planning to apply for Canada citizenship in the future. So, you need to figure out what name you would like to have in the long run especially for your upcoming Canadian passport.

The other option you have is changing your OHIP again to match your current Philippine passport (I hope that will be possible), and then update your PR card accordingly. That way, you will have the same name everywhere and it will be as it is on your Philippine passport. Your passport is probably the most 'important' document among all these, so if your Philippine passport says ACD, then your OHIP and PR card should also be amended and changed to ACD. Forget your original maiden middle name if you would like.

This is kind of a complicated situation and look for expert opinions before deciding which way to go

You need to use the name in your passport, as stated in the application. It's standard to have documents with maiden and married names, so that isn't an issue.

Yes, I was able to revert back OHIP to my maiden name yesterday. Now, I am planning to undergo legal name change so I can change my middle name and go with ACD same with passport.

I'll call immigration to ask if there'll be issue if I go out of Canada when I haven't made the change yet. Also, I can get an Affidavit of One and Same Person from PH Embassy. It might help me along the way to have it on hand.
