According to the medical examination instructions included with the IMM1017, 4 photos. But check with the office conducting the exam -- IME in Kitchener, as an example, takes the photos there electronically.

probably a week or two after the medical is passed they will either issue AIP or a DM. Assuming you have SA already...

SA - Sponsor Approval (the person can sponsor a spouse and (or) dependent)
AIP - Approval in Principle (the PA (Principal Applicant) is eligible to apply for a PR)
DM - Decision Made.
