yes, the child needs to be listed in the application as an "accompanying dependent" and will need to complete a medical exam. this is very common.

It should be your name as you are the other parent and that your child will be travelling with you.

I have a question about medical, maybe you guy can help me. We are doing the final review it catches our attention to medical. Do we need to include the medical result to my application or should I wait for the immigration to send me the 1017 form? I am currently here in Canada. Can I do my medical here in Canada? Should be an accredited clinic by CIC? Does my son need also to undergo medical? He is in the Philippines. Thanks.

Yes, you can do your medical here in Canada. As long as it is a CIC designated medical office, it's fine.
