How long did it take from submission to receiving the medicals request? I submitted my wife's spousal application recently and was hoping to know how long it usually takes for the process.

It depends but usually 3 months or more based on our experience 3 months received medical request but there are some after 5 months or more it really depends. We are Inland app.

Hello I am a Canadian Permanent Resident. My wife live in India. Can I apply for Open work permit for her? Or II have to apply PR spousal sponsorship only?

No - you can't if she's in India.
She would need to travel to Canada and then you would need to sponsor her inland and include an open work permit with that application.
So you mean she need to come here by visitor visa and then I can apply for work permit?
First she would need to come to Canada on a TRV.
Then you would need to submit an inland application to sponsor her as your spouse and include an open work permit with this application.
You cannot submit just the open work permit. And she must be physically in Canada.

If you want her to come to Canada as a visitor then she needs to apply for a TRV. Yes, it's possible to get a TRV during the pandemic but you should expect that everything will take much longer than normal.
Yes, you need to apply for inland sponsorship in parallel with the open work permit. You cannot apply for an open work permit separately.
