My dad didn't want to come to Canada, I am just wondering in the interest form, should i have added him as a dependent. Or is it ok that I didn't add him ?

You need to include him as dependent. But don't worry about it in the interest form.
In the actual application you need to include him as non-accompanying dependent. Your income needs to meet the requirement that includes him. He needs to go through medical and police check. And both him and your mom need to pass both.
At the end, he doesn't have to come.
Hmm what's the point of him having a medical exam, if he is not planning to come to Canada like ever ?
The point is once your mom becomes a PR, she can sponsor your dad if he wants to come over one day, eventually.
And as a spouse, medical condition will not stop him from coming if he is being sponsored by a spouse.
That's why both need to be "clear" before your mom can be approved.
