sponsorship evaluation > employer info, net income and 12-month period

Hi guys, I have a few questions regarding to the employer information, net income and 12-month period on the IMM 5481, the Sponsorship Evaluation:

1. The my partner (sponsor) has two employers: one is a contact centre, the other is Department of National Defence. We provided the employment proof letter from both of these employers. However, on IMM5481, there is only one blank for one employer. Should we write by hand on the side mentioning there is another one?

2. We filled the number of the income according to the tax info and option C, in another word, it is the net income in 2011 tax year. But we filled in the 12-month period as "2011.06.16 - 2012.06.16", which is according to the requirement listed on the form saying "Your net personal income for the 12-month period preceding the date of your application". Is this alright?

3. Since the income is from both employers, I also wrote by hand beside the income number saying "including (both employers)", is this OK?

It asks for the net income for the past 12 months at the time of application - not the past 12 months at the time of six months ago.
If you are writing June 16 2011 to june 16 2012, why would you give the net income of january 2011 to december 2012? That is incorrect information.

On a separate page I would clearly explain that there are two employers, detail the income earned in the 12 months prior for each, and total this to match what you wrote on the form.
