I am Canadian citizen living in the UAE for about 4 years now. I was born in Lebanon, from age 1 to age 16 lived in Abu Dhabi, from age 16 to 33 lived in Windsor, Ontario, Canada and from 2007 to now been living in UAE. I got married last summer in July, I am filling the applications to sponsor my wife because I am planning to go back and live there. I almost completed 90 % on these applications but there are few questions still hanging. Below are the questions I need assistance in them.
I thank you very much and appreciate your assistance

FOR SPONSORSHIP FORMS

FORM: IMM 5540E

Q-8: Some old address I don’t know because I was young and also I used to live in sharing when I came to UAE in 2007, what should I write?

Q-9: What do you mean by Naturalized Canadian? And If I answer yes you ask for permanent resident date? In form IMM 1344E Q-11 you asked the same question almost, I applied for refugee status back in 1990 and approved and become a citizen in 1994, so please assist me on what should I write in both questions?

Q-11: I answered no because currently I am not living in Canada, but I am planning to go back whenever I got my wife’s application approved, and allowed to go to Canada so I don’t know exactly what date should I write in the blank?

Q-12: Are you living with someone? Currently I am living with my wife

Q-14: Some exact dates I don’t remember when my cousins or relative met, what should I write?

FOR IMIGRATION FORMS

FORM: IMM 5490E
Q- 4 and 5: The gifts mentioned there only at the first meeting?

Q-15 and 18: In our culture the reception and marriage are the same, we just go 2 weeks before the marriage celebration party to the sheik to get the official marriage certificate issued.
Should I leave one of them blank?

Q-27: Have you and your sponsored lived together? Is that before or after marriage?

GENERAL QUESTION

What should I write for dates when still present? Like duration of employment and living addresses

FORM: IMM 5540E

Q-8: Some old address I don't know because I was young and also I used to live in sharing when I came to UAE in 2007, what should I write?
Try to find out; if you really can't, then explain in a note that you don't know the exact address and why. Don't just leave a time gap. When you were sharing accomodations, you can still write that address down.
Yes, you are a naturalized Canadian - it means you became a citizen; that you weren't a citizen from birth.
Q-11: I answered no because currently I am not living in Canada, but I am planning to go back whenever I got my wife's application approved, and allowed to go to Canada so I don't know exactly what date should I write in the blank?
You will have to provide evidence that you will be going back to Canada. If there is ever a question that you are not sure how to answer, you can attach a written explanation of your situation.
Q-12: Are you living with someone? Currently I am living with my wife
Definitely tell them this and provide proof.
Q-14: Some exact dates I don't remember when my cousins or relative met, what should I write?
Give the year and the month if possible. You can add an explanation that you don't remember the exact date.
FOR IMIGRATION FORMS

FORM: IMM 5490E
Q- 4 and 5: The gifts mentioned there only at the first meeting?
Yes. Other gifts can be mentioned in your letter they ask for at the end of the forms.
Q-15 and 18: In our culture the reception and marriage are the same, we just go 2 weeks before the marriage celebration party to the sheik to get the official marriage certificate issued.
Should I leave one of them blank?
Don't leave either blank. Just explain the customs of your culture and describe the wedding and reception you had. You can put "see answer to question 15" for your answer to question 18, or try to separate the two parts of the ceremony in your answers.
Q-27: Have you and your sponsored lived together? Is that before or after marriage?
They want to know about both.

GENERAL QUESTION

What should I write for dates when still present? Like duration of employment and living addresses
You can put the date of the application, or add "to present" or "currently" or something like that.

Hello every body i need assistance in more points mentioned below;

thank you all in advence for your help and special thanks for Canadianwomen and Rola

Q1 - Form IMM 5481:
Do I need to sign that because there is no box for signature? Also same applicable for IMM 0008

Q2 - Form IMM 5482 and/or IMM 1283:
I am sponsoring my wife only and we have a baby coming on June? Do I need to fill the complete Financial Evaluation

Q3 - Form IMM 5406:
Section A: the spouse or common-law partner, does this include me, or you meant a previous marriage ?

Q4- Form IMM 5491:
In sponsor checklist you asked for sponsored person passport and police certificate also you asked for it in Part-3 (Country specific) does each of us have to provide separate copies

Q5 – Part-3 of the package (Country Specific):
The two attached DECLARATION; my wife don’t have any children or previously married, does she has to fill any of those declarations?

Q6 – Part-3 of the package (Country Specific):
MILITARY SERVBICE; my wife never served in a military, If she says no she needs explanation, what should I write?

Q7 – Part-3 of the package (Country Specific): Section 5:
-- PROOF of relationship; we both lived in the same country (UAE) prior to marriage and our mobile phones are prepaid, so I don’t have proof for these points, but I provided enough photos of the engagement wedding and mixed once. Is that sufficient?
-- Proof that you are partners; I provided translated Marriage certificate, does my wife have to provide the same or mine is enough? In another meaning does each of us have to provide separate copies?

Q8 – My wife is pregnant, does she has to do the Medical Exam now? Or once Canada’s immigration requests it?

There is no place for a signature, so you don't have to sign them.
Q2 - Form IMM 5482 and/or IMM 1283:
I am sponsoring my wife only and we have a baby coming on June? Do I need to fill the complete Financial Evaluation
No.
Q3 - Form IMM 5406:
Section A: the spouse or common-law partner, does this include me, or you meant a previous marriage ?
It means you. She should list you in A as her spouse, and then not sign under section A.
Q4- Form IMM 5491:
In sponsor checklist you asked for sponsored person passport and police certificate also you asked for it in Part-3 (Country specific) does each of us have to provide separate copies
You only need one copy of any documents asked for (unless the notes specifically say to send more than one copy, which I don't think they do anywhere.) So just send one copy of your wife's passport and the police certificate. The sponsor does not need to send a copy of his passport, unless you are using it as part of your proof that the relationship is genuine.
Q5 – Part-3 of the package (Country Specific):
The two attached DECLARATION; my wife don't have any children or previously married, does she has to fill any of those declarations?
These 2 declarations are only for citizens of Russia and former USSR states, or residents of the same. Your wife doesn't need to include them.
Q6 – Part-3 of the package (Country Specific):
MILITARY SERVBICE; my wife never served in a military, If she says no she needs explanation, what should I write?
This is here because in some of these countries military service is compulsory, so if you didn't serve, you would have to explain why. If your wife is a citizen of a country where military service is not compulsory, she can put "N/A" on the form, and write as an explanation that in her country women are not required to do military service.
Q7 – Part-3 of the package (Country Specific): Section 5:
-- PROOF of relationship; we both lived in the same country (UAE) prior to marriage and our mobile phones are prepaid, so I don't have proof for these points, but I provided enough photos of the engagement wedding and mixed once. Is that sufficient?
-- Proof that you are partners; I provided translated Marriage certificate, does my wife have to provide the same or mine is enough? In another meaning does each of us have to provide separate copies?
You only need one copy of any evidence you send. The sponsor's application and the applicant's application stay together throughout the process, so you can attach proof that is asked for in both applications in a separate section at the end.
To prove your relationship is genuine, send in as much evidence as you have. Since you have been living together, make sure to send proof of this: both names on a lease or mortgage, an affidavit from the landlord, roommates, relatives, etc. stating that you two are living together. Utility bills in both names. Joint bank accounts and credit cards. Mail addressed to both of you at the same address. Etc. Anything that shows you live together.
You can also get affidavits/letters/emails from friends and relatives stating that they know your relationship is genuine because ...
Q8 – My wife is pregnant, does she has to do the Medical Exam now? Or once Canada's immigration requests it?
Because it is only valid for certain period of time
She should not have an X-ray while pregnant. Somebody on the forum said that CIC accepted their medical without the x-ray, and just had her do the x-ray after the birth of the child. Other people said they had to wait until they gave birth before they got the whole medical done. Maybe try phoning the DMP you will use and asking him/her what she should do.

This is why they do the urine test

When signing the forms, some forms ask for City and province? What should i write there?
for example my husband took the citizenship from ontario and he lives in ottawa so i wrote those two, if you are going to toronto for example write toronto province: ontario. so write where do you want to live

in Appendix A of the Part-3 "Country Specific Instructions"

1st Requirement "IMMIGRATION FORMS"
They ask for Supplementary Information Form, what is this?
the supplementary form is the form existing in the appendix A : the travel history, education and employement history, military service
