sponsor questionnaire IMM5490

hi all
i just want to reconfrim who will put detail on this form IMM5490,
because my check list showing this (if your sponsor is your spouse he or she must complete sponsor questionnaire)
so pls tell me i or my sponsor wife will complete this form some ppl told me before this form must be complete by me.

The directions for the questionnaire are: You are to complete this form if your sponsor is your spouse. In other words, you would not complete this form if your sponsor was your mother or father or child, that is what your checklist is trying to make you understand.

This is a "sponsored spouse" questionnaire; meaning the spouse of the sponsor. If you are the one being sponsored then it is you. It also says that this form is to be completed by persons living outside Canada and being sponsored as a spouse: would that be you?!
