My wife (the sponsor) is Canadian born and Canadian citizen, I (applicant) got pre-arrival and I sent my passport to Vienna today, but we want to live in my country for couple of months (3-4) and then go to Canada together. Will she lose sponsorship or will that be a problem, do we need to let Canada visa center know about that? My medical exam is expiring in 7 months.

If you applied inland then yes your application could be refused. If she is the one moving to your country not a problem but she needs to show you will move back when approved.

That's fine. She won't lose her sponsorship and you don't need to let IRCC know if it's after approval. Just make sure you land before COPR expiry.

Passport already sent to Vienna, so it's outland and already approved. No need to notify IRCC or prove intent to return
