For IMM5481, on page 1, items 4-6 total is 3 for my case (principal applicant - my wife, me, and our son who is already Canadian). On page 2 of this form, I have answered "NA" for all but number 6B) I entered our son's information. Is this all correct?

Regarding forms that don't apply to your situation, here's an excerpt from a Q&A session I had with the CIC call centre.

You should send the forms even if they don't apply, just so that they know you acknowledge those forms. If they don't apply, you should put a diagonal line through them and write “Not Applicable” on them. Another solution could be to write an appendix to the Document Checklist and explicitly state that you are not including those forms because they are not applicable to your situation. Or you can do both.
I ended up sending a written appendix to the checklist (IMM5491) that explained in detail what was submitted and why. As long as you can explain that you've included everything you believe is necessary and why you're not including things that aren't necessary, you should be OK. If you're submitting supplementary documents that aren't strictly required in the checklist, you should explain why you're sending those, too. When submitting everything, I just put all the papers together in order, with the checklist, appendix and supplementary documents at the front, followed by all the rest of the forms. Specifically, here's what we submitted--note that we submitted in June 2011, before they switched to the newer versions of the forms (esp. 1344).

I added the "evidence list" and "photo totals by envelope" because of the huge amount of evidence we submitted, just to facilitate the job of browsing through it. You probably won't need to include those in your case, unless you happen to have submitted 100 pages of receipts.

They were very clear to me about NOT stapling, using paperclips, or otherwise binding any of the pages together. They literally want everything loose. You can pack everything into a manila envelope and the people who process your file will be able to keep everything in order.
