Some queries for medical exam done at Nationwide in Manila...

good day also to all!

I have some queries regarding the medical exam at Nationwide, i hope u could give us some info with regards to it.

Me and my daughter who is 8 years old is planning to have our medical exam next week at Nationwide. Our papers were sent by my husband in Canada under a Family/Spousal Sponsorship at CPC-M on May, and it was received on the 26th of that month. July 14, our package were sent back to my husband with the client id-num and kit id-num with the letter stating that we need to attach the medical result as well as the receipt for the payment of the processing fee. The letter indicates also that they will be giving us 90 days to comply with the requirements needed. I asked my husband if no such additional documents were requested on the letter. He said "none", only the medical and payment for the processing fee is required before they will gonna proceed for the processing of our application.

My questions are: Upon going to Nationwide Health System Inc., are we need to bring any letter or proofs for them to know that we need to undergo a medical examination for the sponsorship application? or we could just go there and request for medical exam? Also, aside from the photos and passport, what else do we need to bring? And how much is the fee for the principal applicant as well as the fee for my daughter. Hope to hear from you guys soon.

You have to go to a DMP - a designated medical practitioner - for your medicals. This website has a list:

This is what the Immigrant's Guide tells you to do for the medical exam:

"Step Action
1 Make an appointment with a Designated Medical Practitioner (DMP) for you and each required family member. DMP in your region: http://www.cic.gc.ca/dmp-md/medical.aspx

Fill out Appendix C and bring it and the following items if applicable for you and each of your family members to your medical appointment:

* Appendix C
* Passport and photocopies of the biographical data pages (this is the page that shows the date of birth, the country of origin, etc.)
* If no passport is available, provide an official identity document bearing a photo and the date of birth of the family member
* Eye glasses or contact lenses
* Previous medical reports, prescriptions or treatments
* 5 recent photos, taken within six months preceding the date of your examination
* Immigration client identification number (ID #), if known

3 The DMP will give you Copy 2 of the Medical Report: IMM 1017 EFC - Medical Report Section A. Include the original of Copy 2 with your application. (Photocopies will not be accepted.)
4 The DMP will send your results to the appropriate medical office for processing."

I'm not sure what the fee will be.

About the payment of fees, there is a form attach into the letter, my husband told me that he will be paying it to the bank. By the way, can i ask how much did u pay for the medical exam? me and my daughter need to undergo the medical.

It's 4000 pesos. I think it's the same whether it be a child or an adult.
