No, I didn't. Even in CIC application, there is no such document required in document checklist.

Just how recent do the forms we sign & date have to be? I mean the federal forms to be uploaded on OASIS.

Do they have to be on the same date you upload, same week, same month ??

a question for u.its about merging the documents.
well,I have my birth certificate in local language,
I have translated it in English with a Notary public
.Now I have 3 documents,
original birth document in local language
translated in English birth certificate
and translator affadavit.

As per my understanding,from ur advice
do these separate documents be merged in
one??or the merge is for the forms?plz

No need the same, put the date you sign. But the date should not be outdated with the submission date (<=1 month to submission date is fine).

I am interested to apply for SINP nomination and I already filled up the OASIS application from but I can not find any option to upload my wife's and Child details, child details, my parents and spouse's parents birth certificate.

Also I can't find place to attach the settlement plan.

I would really appreciate if anyone can advice me how to upload those documents.

No need the same, put the date you sign. But the date should not be outdated with the submission date (<=1 month to submission date is fine).

Merge as a single pdf doc named birth. When open birth, then all 3 would open serialwise as first original copy, then below second eng. Tran., afterthat below trans

Dear Hasan,
once you ll complete all the sections,14 sections in OASIS
it will lead you to the another part ,where u ll be able to
upload those forms and detail of ur spouse n child,I had the
similar issue prior.all d best,Hasan.

After completing 13 sections in main screen (all sections are in green color), click section 14 Applicant Details-Final Page to lead you to the screens of family members, settlement fund, settlement plan,etc. Try to practice more and more, you ll be familiar with it. I started 3 weeks b4 submission date just to be familiar with the tool, fill infor, upload, re-upload, etc.
