Hi Goodman36, I found Appendix A is a checklist and two forms are for Srilanka and Pakistan. I did not see anything for Bangladeshi Applicant which we need to send with passport. May be I am wrong or may be I saw the wrong form. I think SVO sent you a link with your instruction. If you don't mind can you please send link which SVO sent to you?

Hi man: It should be under 'Applicant' column. because your spouse/wife is the applicant. You are sponsor. This form is not available online.

What did you 'tick' for the option of "How would you like us to return your passport ?" * Air Mail, * Registered mail, * In-person ? Which one did you 'tick'? I think it should be "in-person" since the applicant will pick it up from VFS in-person. Am I right? Canadian Emabssy is now completely closed in Dhaka now.....!!!!!

Hi AKHI: But the applicant is dropping the passport at the VFS dhaka office. Once it will be returned to VFS, then you are supposed to go to VFS and pick it up... Am I right ? Lets see what goodman has to say...

Sorry! man I mis-read you last time. Answer is NO. You name should NOT be anywhere of this form. ONLY your wife's information and in the APPLIACANT column. Hope you got it now.

Hi Man: Once again, if you read the FORM at the top it said " Do NOT send passports for non-accompanying family members". So you are NON-ACCOMPANYING member in her PR application. In short, they need infos only of the passports will be sent with this FORM. Hope it helps now.

Yes, if you are using VFS to send the Passport to SVO, then you should check this option. SVO asks that Bangladeshi Applicant use this option even though the other two option 'Registered Mail' and 'in Person' is also available.

Even though it says, "Do Not send Passports for non-accompanying family members", it does not mean NOT to fill out the information. That is why I am confused.

I got the Form (Appendix A) with Passport Request Letter that I received by mail from SVO.

It is not related to application submission or country specific forms. It is an Appendix that asks to fill-out the information of "Applicant, Spouse, Children" on a Form and this is to be sent with Passport.

Hi man: So what I understood from your post you are checking the box *In-Person. Am I right ? Box for "Registered mail" and AirMail" is NOT valid for your case since you are dropping passport to VFS. Please confirm in reply to us.
