Hi, I have a half sister from a previous wedding of my dad but honestly I have seen her 3 times maximum and I lost contact with her 20 years ago. What should I do with respect to the family information form, if I only have her name for sure but nothing else?

Maybe write about it on the Letter of Explanation (cover letter). Mention to the Visa Officer that you want to be honest about your family history but unfortunately you have no details about your half sister.
