Most forms, paper applications at least, when filled out in capitals and black ink are clearer especially if need to be scanned or photocopied at any point in any process.

PRINT NAME is simply defined as writing your name in CAPITAL LETTERS! Unlike Signatures that are mostly written in cursive or scribbles, thus making them hard to read, PRINT NAME simply demands that you write very clearly and without connecting theletters, So your writing looks like Printed Text!

Assuming you are filling out a paper form (and not one online), then "print" means to write without joining the letters. The letters do not have to be all capitals. In this context, printingis opposed to writing in cursive.

Thanks for sharing this. However, does that mean to leave space between the characters?
 
Not required to be in upper case. If you want you can only print names in upper case if you want.
