For my 5-year eligibility, I have spent the period of 3 months in my home country working remotely for my Canadian employer (vacation + work). I have listed this duration as days spent outside Canada in the physical presence calculator. Should I also list my address in my home country for this period?

If you did not have residence in Canada during that time then you need to mention it. Otherwise it's not needed.
