Should i declare that i applied before?

We had a rejected application where my wife was the main applicant.

We then took the ielts and and i had the better score to become the main applicant.

In the section where they ask if we applied before should i mention yes? Since they ask: did "main applicant" apply before?

Even if you were a dependent but you were denied visa before and you have to mention that in your application.

My former consultant attached my degree certificate twice. In its place and in the place of the police certificate so we were denied for missing police certificate.
