no the spouse doesn't have to do medical and pcc, only the principal applicant has to go through medical test and PCC

Actually, if a spouse is listed as a dependent on an application, he/she does have to do medicals and pcc...

but my spouse isn't applying for PR he is my sponsor i am the only principal applicant so shouldn't it be only 1?
Is your spouse a Canadian citizen sponsoring you? If so, then it would be just you.
Perhaps also try a different forum

Okay, if your spouse is sponsoring you, that is the spousal sponsorship programme. That means you are applying for permanent residency under the spousal sponsorship programme, not under the Express Entry programme.

This forum is the Express Entry programme forum; persons here generally only have experience with Express Entry, not spousal sponsorship. If you find a spousal/family sponsorship forum, the persons there will be better able to assist you.

Correct. She has to do only do medicals for herself.

I received a letter from cic to fill form IMM5562e (Travel History), should I include anything with it, or just filling the form will suffice?
