If you have applied for Visa or work permit earlier, in the WP/Visa forms there used to be a page for residency history. It was a table with columns - From ( MM/YYYY) , To (MM/YYYY) , Address , total period in months
Just create that table on a page, fill the details , write one liner declaration stating that above information is correct , sign it and put date and place. Scan this page and attach it to the email reply. Write your information i.e. UCI, application number , it will be also good if you can write same details as on the scanned page scanned page into email body.
I think that should be sufficient.
