Separated Status

Someone could please guide a candid suggestion on the "Separated" status ?

I am living separate from my wife and my kid from the last 6 months and want to file PR on Express Entry for Canada.
I want to apply for Canada PR individually so, do I need her signature on the PR forms ?
If yes then is there any way I can escape from it ? through any court affidavit...

You mean you left your wife? To get a divorce later? You do not take her to Canada (in terms of getting PR for her)?
Then if such a case she does not need to sign anything BUT you have to show your marital status as separated. To wrire her in the forms - as your "spouse", and you kid MUST undergo medical test (without this test you will not be able to sponsor him to come to Canada EVER).

as you said I need to mention myself as Separated
Do I need any Self-Declaration Or any Court Affidavit to mark myself as "Separated"
