A question that may or may not be of sense with others but still I would like to get some advice and answer from anyone :-)

It says on the instruction that "To indicate your name, the form's title and the number or letter of the question you are answering on all additional sheets of paper on which you will provide your answer." I'm just a bit confused. Do I need to have a separate sheet for each of the questions that would require me to do so? Or, as long as the space is still big enough, I can continue and add another question/answer with the current sheet?

I simply had one separate sheet attachment where the space was insufficient on IMM 5490. Basically I put the IMM 5490 reference and 'appendix' in the 'header' of a Word document, together with my wife's full name. I put the 'page x of y' in the 'footer', then I put the question number and repeated the question in the body of the document, followed by the additional information, repeating this for each question that required additional information that wouldn't fit sensibly on IMM 5490. On the last page of the attachment, I created a copy of the applicants signature and date block so it looked like IMM 5490.

So I ended up with one 'official' IMM 5490, completed, signed and dated (with refer appendix where appropriate) and one Word document appendix which had multiple sets of additional information for multiple questions - hope this makes sense.

You don't need a separate sheet for each question on the same form. However, if you have multiple forms with additional answers, each form should have a separate sheet.

Appreciate the replies, Hurlabrick and canuck_in_uk!

Hurlabrick, my header would be as per what the instruction says. There are a few questions in IMM 5490 that would require me to use a separate sheet, thus my earlier post. Some answers though would not be too long but not short enough to fit and answer directly on the form itself. For a separate sheet that's still showing sufficient space, I just continued and added a new question no. with my narrative answer. I did put a page no. but will change it to read as 'page x of y" like you did :-). I think this is a good idea. I just got confused earlier as I was told that I should have a separate sheet for each question (IMM 5490) that would require me to do so.

canuck_in_uk, I will be having separate sheets only for IMM 5669 (1 sheet) and IMM 5490 (4-5 extra sheets).

Thanks again to both of you :-)
