Dear All

Along with the medical request I have also received a set of forms (total 4-5) called Immigrant Summary Form:

In Instructions it says (It says must be completed in duplicate for all persons over 18)

This form has date for principal applicant (me) spouse and dependant (child)

1] First Question is I have already filled this form with our datas, now should i take another copy of this form and fill it again with my spouse being principal applicant ? (data will be indentical ) except that the information in the columns will be reversed.

The subsequent pages of the form have a questionaire to be completed by all applicants aged 18 or over --

This I understand has to be filled by both me and spouse because it will get different responses (data).

2] I also want to know whether to fill in information for our 2 yr old child in the form or not - - ?

3 ] I guess the main confusion is that _ I applied for immigration so i assume im the principal applicant -- but in immigrant summary form (do both me and my wife quality as principal applicants - and have to fill our own separate forms ?)

You are the principal applicant
Your wife is your Spouse
and your child - Dependant child.

Make a photocopy of each sheet and fill the details asked in both sets of form. Your wife need to fill the forum as your accompanying family member - spouse - and not as principal applicant. Your child need not fill a form because of below 18 years,

Wilson i dont think ur correct

I checked with a senior consultant and he said Both me and wife have to fill form separately, in one case im the principal applicant and in the other she is, the order will be reversed

Suppose i had a 19 year

Then obviously the 19 year old would have to fill his spearate form as the principal applicant

That is what I replied already. Did you read :

Make a photocopy of each sheet and fill the details asked in both sets of form. Your wife need to fill the form as your accompanying family member - spouse - and not as principal applicant. Your child need not fill a form because of below 18 years,

Perfect.
But in Form 2 when she fills, your name should be shown as her spouse.
In fact this form is making confusion for lot of people because of bad formating. Even though you give details of wife as Principal applicant on this form, you will be the Principal applicant for the Immigration.

Its the fault in the wording of the form that puts wife as principal applicant even if she isnt !
