send passport to LA but forgot passport request letter

I have sent my passport to LA yesterday, but I just realized I need to send the passport request letter with my passport.

I am wondering if I should sent the letter right away and a copy of my passport to explain this situation by mail.

Did anyone have similar experience?
 
Have you sent any kind of a note with passport ?

If yes then i think it should be fine.

I just sent my passport, photos and a piece of paper with my name, height, and eye color.

For the note, do you mean a note to explain that I received passport request letter, and sent my passport to them based on the instructions? Unfortunately, I did not.

I also did not send passport request letter with passport.

I am sure they will figure it out
 
I didn't send in any passport request letter with mine. As long as you used the original document they provided you when you filled out the height, eye color, etc. your file number will be listed, so they will know it belongs to your file.
I also included the file number on the envelop so they knew which application the documents belonged to. I wouldn't worry too much about it though.

which original document did they provide for height, eye color etc? I didnt receive any of that
 
When they sent me an email notifying me for passport request, they included a form that requires you to fill out your height, eye color etc.. It's titled Fees and Passport Instructions.. That's the only form I submitted with my passport request. It took exactly 2 weeks to send and receive my passport back (from Toronto to Detroit).
 
I got my PPR from Buffalo and they did not inclue a form. I simply put a written note with my height and eye color when I submitted the passport.

It should be okay. How did you know to include a note with your height and eye color then? Did you use registered mail for your passport return? You can track it online next week and see if it's on its way back from Buffalo.

Because the instructions they sent with PPR stated to include height and eye color. I think next week will be too soon as the passport was delivered at Fort Erie on Monday this week. Not sure when it will be delivered to Buffalo.
 
I'm a bit surprised that all their forms are not standard then.. The fees & instructions form I received listed all the steps to take to mail in the passport, pictures, etc.. and had instructions about height/eye color, etc. with a box for you to fill in.

As well, the letter attachment I received notifying me of my PR visa was a standard document, didn't even have my name on it.

I sent mine to the Windsor address for Detroit, was received on Monday by Windsor... two Fridays later, my passport was back in my mailbox. I used registered mail for both ways.
 
My wife did the same thing. She sent only the PASSPORT the PHOTOS and a piece of paper with her Name, height and eye colour. Please tell me that everything will be ok! Our visa office is BUCHAREST. Please tell me something positive guys :)
 
Hi everyone, have you gotten your passport back yet? I sent an email to explain my situation when I realized I need to attach the passport request letter three week ago, and they replied me today, said "Your passports will be sent out sometime next week". Hope I can receive it by next week. It seems okay if you forgot to sent that letter together, they may just need extra time to find your file(I guess).
