Selection Certificate documents check list

Hello All,

Can someone list all the forms including check list for Selection certificate of Quebec.
I have submitted my spouse sponsorship file to CIC. After the approval of my application I will have to submit application for CSQ.
I would like to prepare all documents for Quebec selection certificate.

and please explain about the OFFICIAL FAMILY NAME APPEARING IN THE PASSPORT and FAMILY NAME AT BIRTH? are we talking about the same person?

Question 2.2 asks your spouse whether she has a "de facto spouse" while she is married to you. Typically, "No".
Question 3.1 should be skipped (read the little note next to question 2.1) - answer 2.1 and 2.2 then skip to 3.2

All French yes! The only one they accept not being in French is the application for selection certificate.

I sent them all in French to not have any issues.

Oppps, I didn't include that form as in check list it is mention that "If you have divorced or separated, or if you have children from a previous union, enclose this form, completed and signed."

So. I din't include below form.
