based from experience (I had my medical at NHSI Baguio), urine samples are collected in the clinic, they have toilets inside. The clinic will give you an information sheet which you will include in your application. I was also advised to attach a photocopy of my receipt. Results will be forwarded by the clinic to the visa office but they will inform you of your results.
