Sample reference letter - Intro Paragraph

Hello all,

Since My employer refused to provide reference letter, I'm going to request a reference letter to my supervisor when I worked at Canada. But he is not with the company now.
I was working as a contractor at my client location. I'm going to ask him for a ref letter in plain paper since he is not in that company which I was working.

Seniors please guide my to prepare ref. letter. I need a help in preparing introduction paragraph. Can I tell his situation now like he is no more in that company now etc?? or since it wont be in letter head, will cic ask for the reason? Or Can I tell cic that he is not with the company anymore in separate letter?

i would say, just get it on a plain paper and see if you can get this business card.. else have all this contact details and his designtion(while he was with company)

I actually stuck how to write a introduction paragraph in ref letter which i'm going to get from my client.
for example, he has to tell that he is not with that company no more kind of ???
