Right of Permanent Residency Fee Form!

I need help as to what application form i need to fill and send back with a cheque of bank draft of $490 cad for the right of permanent residency visa.

Has anyone got an idea?

No..you don't need fill up any form for the RPRF fee...you can write a simple cover letter providing application number and inform them you have done medicals ..make sure you write your name and address on the back of the demand draft....

Tasnim just a quick question, do i also need to send the receipt that came with the bank draft of tear off the bank draft from it and keep the receipt for my records.

No. You need not send that bank receipt. You only send the Bank Draft as suggested by previous posts.
