Yes the same number on directory, In all probability he will not pick up phone so leave a VM or SMS and he will reply back.I've been trying to find the office number for the panel Physician in West Bloomfield but am having trouble. Called three offices yesterday all of which said he worked there/worked there in the past but didn't do the medical exam at their location. Even got a hold of another Dr Sherman yesterday who said he was retired but wasnt the panel physician I was loooking for. Tearing my hair out here. For those that saw Dr. Ronald Sherman, what was the number you were able to reach? Was it the one associated with the canadian directory?

I did a Google maps search of the address and street view showed a residential home rather that what I would have expected to be a doctor's office. Do you happen to remember where the office is, how long it may take to get a response and schedule an appointment, and what the estimated costs are? Sorry for all of the questions! I have Blue Cross Blue Shield of Michigan insurance and they covered most of the xray, urine test, and blood work.... It was $220 and I paid $33 after my insurance covered it. The trick is that they billed it under the following billing diagnosis code: Z111 (Encounter for screening for respiratory tuberculosis). Technically it's not considered preventative care but it was covered under my insurance.Hello,

The trick is that they billed it under the following billing diagnosis code: Z111 (Encounter for screening for respiratory tuberculosis). Technically it's not considered preventative care but it was covered under my insurance.
You can call you insurer to check your coverage ahead of time to see if that diagnosis code and procedure codes are covered. If your insurance comes back telling you that you need to pay more, check what billing codes they used in your claims, you can always call the hospital and have them resubmit under a different code if they didn't use Z111.
Because the medical exam from Dr. Sherman was in cash ($200 I think) I didn't charge it to my insurance.
I got mine done at Costco. The lady at the counter said she doesn't have any specs for Canada photos, so I showed her the required dimensions and she said Australia photos have the same dimensions, so I got'em done with those specs. Just checked Australia photo requirements at https://www.passports.gov.au/passports-explained/how-apply/passport-photo-guidelines and these dimensions would cover what you'd need for Canada. So, I'm guessing mine will be accepted. Make sure you ask for a digital photo, though! I got 4 printed photos and one digital photo (in a complementary flash drive) for $9.99Hello
Wanted to ask you where you got your photographs done? In our area I am not able to find someone who does passport photo with Canada specifications.
