please dont panic.
yes you can apply for restoration for you status. I dont think you need new lmo since you are with the same employer. just send all the documents eg. new passport ( you can send old canceled passport copy too), old work permit, contract signed and updated which means you and your employer sign with the new date and all other required document and a cover letter why you lost your status and on what basis you want to restore your status explaining detailed situation. I think you should be good. But you cannot work until you get your new work permit.

I have a question can someone on open work permit can work as caregiver with no prior experience or any course certificate. If yes then what are the requirements because the open work permit is set to expire in sept 2015
The work permit cannot be renewed without an LMIA and I heard in order to get caregiver work permit applicant must have either an one year experience or 6 months certification.
