I went for my citizenship test and scored 19/20
Interview ... Officer isn't satisfied with residence proof n asked me to send health record for the time mentioned in application.. I was healthy enough to go see a doctor so don't have any record on Ohip
Please advice what else I can send
Lived with family so don't hv a tenant lease agreement or any such proofs either
Sent all t4 and NOA already
Plz help
I have to submit these docs within 30 days or else they will abandon my file

This is confusing. You seem to be at an advanced stage - citizenship testing etc.

Then why does your profile on the side bar by the posting say CPP-Ottawa etc, and that you landed in 2014, .

Credit card, bank account statements; utility bills on your name, insurance, etc should work.

If you don't have any of that, probably, you can ask witnesses to confirm your presense (but that's probably more complicated).

Since they asked specific documents (which is replacement of total RQ) and you don't have it. Since you have the OHIP card and you never visited a doctor in last 4 years that could be tricky in terms of CIC thinking. You are truthful but they will think otherwise. If you would have seen a family doctor OHIP record could have been updated regardless no. of visit and even one or two visit would have been justifiable that you are healthy no frequent visit was required.

However, you have to response within 30 days. In may opinion, go to your local family doctor for a checkup and explain the doctor that CIC requested record but you were not required to visit a doctor as there was no complain. Request a letter from Doctor mentioning your healthy status is fine as you not required to visit doctor in past 4 years.. Write a letter explaining the situation attach the doctor letter and submit..see what happens next.. If you have other residency proof may enclose with as an optional.

at the interview after that test the questions they ask are related to the 4 period or since we became PR?

The history of visits is not the only thing why CIC asks for the OHIP records. I read somewhere that it is also a proof of having a valid OHIP coverage during the period in question since the validity dates of OHIP card do not mean that there is valid coverage.

Therefore, my recommendation is to get the OHIP records as proof of valid coverage even if there is no visit history.

The letter you get in that case does not state that there was valid coverage during that timeframe: "A record of the individual exists, however, no OHIP claims were processed against the health number provided, during the time frame requested."
