There is no specific format for a reference letter. You need to get the following details on your reference letter:
1. Position worked as in the company from "Start date" to "End date"
2. No. of hours worked per week (min. 30 hours per week or above)
3. Roles and responsibilities for this position
4. Annual salary for this position
5. This reference letter must be on the letterhead of the company and signed and stamped by the authorized person (Your manager or HR or Director of the company).

Some companies do not provide salary details on reference letter. In that case, you can attach your salary slips or work contract which has salary details as proof of your salary.

You mentioned minimum 30 hours per week. Correct me if I'm wrong, but someone who has worked on average 15 hours per week for 2 years or 7.5 hours for 4 still satisfies the work experience requirement.

I am not sure about the exact format. but I found this on CIC may be this will help you:

A reference or experience letter from the employer, which should be an official document printed on company letterhead (must include the applicant’s name, the company’s contact information [address, telephone number and email address], and the name, title and signature of the immediate supervisor or personnel officer at the company),
should indicate all positions held while employed at the company and must include the following details: job title, duties and responsibilities, job status (if current job), dates worked for the company, number of work hours per week and annual salary plus benefits

I worked during my ph.d and it was about 35 hrs per week for 3.5 years.

The responsibilities for RAs are not lot. May be 4 or 5 in the NOC description. Just make sure your reference letter has 80% similar responsibilities as in the NOC description (not word to word copy though).

If you have worked for 2 continuous year for 15 hrs/wk it is fine. 2 years mean 104 weeks. If there were paid leaves thn that's fine. However, if even for 1 week you were on unpaid leave then u r not eligible

No, I don't think so. You would still be eligible.

Could you please provide a clear reference for that because, as per CIC a short break in between employments is fine, and in this case, there's no money earned nor is that person employed. So logically, a two-week holiday because the workplace closes and not because one's professional activity has ceased should not be counted as a "break in employment" as that person is still employed, still has a payslip for 12 months. If someone more knowledgeable could weigh in.
