Hello folks,

I am not using a representative and also do not have a Common-law union. I am not sure what to fill in these forms or do I just send them blank with my application package? Please let me know. Thanks.

•Use of a Representative [IMM 5476] (PDF, 55 KB) >>>I do not use a representative.
•Statutory Declaration of Common-law Union [IMM 5409] (PDF, 19 KB) >>>I have a spouse but not a common-law partner.

If that is the case...you just dont need to send those forms.
Or it at all you are sending those forms then you mention them as Not Application in the form as well as in the checklist.

I dont think there are any guys here who are using a representative for immigration services....So i think most of them have not attached the form and must have written in the checklist as Not Applicable OR send the form with Not Applicable written all over it and also in the checklist.

For representative, in checklist specify that no representative ( or tick option in form)
