I have been requested additional medical test, sputum and x-ray after 3 months. Is this common ?? I am really worried.

Not common, but not that unusual. Most likely is that you have had tuberculosis in past or latent tuberculosis. It won't harm your application, but there may be medical follow-up required in Canada.

Don't worry at all. It's not that common to get that but it's an easy process. You get those additional tests done and then a follow up xray when you land in canada.
