Hi, already got my extension to stay here in canada until april 2015. We are planning to go to USA for vacation for 1 week on August, my question is... do i need to get a request form to enter to canada? i have a single entry visa and im a spanish citizen. where can i apply for it and How long does it takes to get it. Thank you so much!

No, you are from a visa except country. Just bring documentation showing reasons to go back to Spain at the end of your trip. You likely will not have to show them. If your asked for anything it will be a return ticket.

hi, im not going back to spain, i will go to USA for 1 week vacation. im here now in canada as a visitor and all i have is a paper that it says i can extend my stay here in canada until april 2015. i want to ask if i need to ask for a "request to return to canada" to enter again to canada after my 1 week vacation in US. thanks!

You are totally fine with going to US and coming back with a single entry visa. Also, if your or anyone else holds a study permit you are allowed to enter Canada from the US just showing your valid SP even if your entry visa is long expired.

You have mismatches in your statements.

In your first post you state that

And in the second you state

i have is a paper that it says i can extend my stay here in canada until april 2015

Your two statements are different, and each require deferent response.
