Its basically a london visa office specific form, which require details of previous passports held, foreign travels, and background history for previous ten years. Some of the info will be duplicated in the imm008Sch1 and generic forms, so pls check the copies of the forms you have submitted to ensure the dates and details tally. Its for CiC to carry out background and security checks, along with the ACPO certificates, and its nothing to worry about.

You'll find more info about it on the london visa office threads, and the sticky threads at the top of the forum. Messrs Cohen have also drawn up some very useful FAQs on background and security checks on the Canada visa homepage.

WOW thanks Oceana, Verry useful info, but is that a good sign for my application i mean requesting these 3 things.

Thats my timeline

Initial Appl to CIO= 9 MAr, 09
AOR from CIO= 21 Mar, 09
Full documents to London= 16 July, 09
AOR from London= 31 July, 09
e-cas as 'In Process'= 21 Jan, 10
Medical request received=25 Feb, 10
Meds done= 5 Mar, 10

Ofcourse, it certainly is. You're making progress. RPRF request is them asking you to pay your visa/landing fees, Meds is to be sure you won't cause excessive demand or pose a health risk to the Canadian public, and that is a good sign. Clear your meds, cut the drafts & forward to them, and furnish them with your travel/immig history and you're looking at your PR.
