Request for IMM 5669

Upon submission of PR Application, I received a request for IMM 5669 today through myCIC (http://www.cic.gc.ca/english/pdf/kits/forms/IMM5669E.pdf)

Is it normal to receive the request to submit this form ?

You cannot have any gaps in your address history so make sure that you can account for all times.

little background before my query

DOB - October 1997
turned 18 - October 2015
In PCC, the time period is
01-Aug-2015 - 26-Dec-2023
In address history, i have only 1 address whose starting date is
10-Sep-2015

In personal history, I have provided history from Aug-2015 till date

ITA - 09 Dec 2023
AOR - 05 Feb 2024
Medical & bio - 12 march 2024
ADR (IMM5669 Schedule A Background Decl form) - 22 April 2024

I am worried why i have received this ADR and how should i respond ? I have to reply back before 28 April 2024 Would really appreciate any help/guidance
