I made another document on a note pad with all the names in native language with the corresponding english names next to them.Uploaded the jpg along with my application form to cic with LOE explaining why i have written the names in native language in a seperate doc.I did this cos the application mentioned PRINT(in bold) names on it. Didn't want to take any chances. My Application was accepted.

Is the form required upfront or when it is requested? I feel all the information is already submitted through the electronic portal...if some one gets a request to fill it only then they fill?

for the Native language in Form. 5406, Do I need to include Hindi on the same form or need to attach another seperate for with different language. Does native language has be fill in PDF only or can download and fill by pen as well?
