I have got the below request:

A newly completed Government Service Details form (enclosed). Please provide a completed Details of Government Service form detailing the specifics of your duties in your government position. Ensure to provide the information in chronological order and with NO gaps in information. You may use multiple copies of the form in order to provide all applicable information.

Can someone please guide me on how to respond to this. There was nothing enclosed along with the letter. Which form do I need to fill in ?

You need to include more details about your government positions.

I think if you provide this type of letter, it may take ages to get a response and even then who knows what it'll be (they may ask for resume or c.v.). I'd suggest responding with actual content as to what you did. You can still include the text you have above in some form, but add something to effect that "I am providing a summary of my government service and I hope this may be sufficient to respond to your request, but I remain ready to complete the government service form referred to."

There is a military service form. If your service was armed forces or any paramilitary (police, security ministry, etc - even if 'civilian' position but in something like interior or defence ministry) I'd use that. If anything you feel needs explaining, separate/attached clarification.

If your position was not in any way of this type, I'd use this form as a guideline to set up a table and provide the info as best you can. Adapt as best you can i.e. rename the columns to relevant names; omit the parts that clearly arent' relevant. Be clear if any positions were during periods of conscription / alternative service (i.e. if you did those positions as part of / in lieu of obligatory service). If you were required to do any of these positions as part of education, indicate that somehow.

If you also have a prepared resume, I'd attach that as well as the form.

For future reference, it is called "Details of Government Employment - IMM0149".

When returning this form, I'd include a short note to effect that the form was not attached, you are providing a completed IMM0149 on assumption that this is the form referred to in the letter.
