I got the same request. Not sure why, but I put about thirty trips into the post-ITA form, and maybe they had some bugs and didn't get all the information. Or maybe they didn't want to count days themselves (in 5562 you should do it). Anyway, don't worry, just fill the form and submit it.

Thanks! So for the original form, did u write the info or type the info? Thanks.

I am asking because the original form does not show date. So I plan to print the original form and handwrite the trips, while for the extra sheet, I would type and print. Any ideas?

How long did CIC give you to respond back with the travel history form?
We only got 5 days - is that also how much you got?
