Hi new to the forum.

I have a child passport which has expired for more than 10 years (I have not been living in Canada for the last 10 or so years). I would like to get my first adult passport. (Potentially considering settling in Canada for good now that the UK has voted for Brexit!) The trouble is that I have legally changed my name since then in the UK.

My understanding is that I need to reapply for a citizenship certificate so reflect my name changes before applying for an adult passport. However, there are certain sections in the citizenship certificate application form (CIT0001E -2) that I am not sure about, particularly in relation to section 8. I was quite small when I got my Canadian citizenship (through naturalization - at the same time my mother got her citizenship) and I have no idea when my mother first entered Canada for example. To be honest I can't even remember exactly when I first entered Canada. I could make an educated guess of a year that might be +/-1 year but I could not pinpoint to the exact date.

My question is that since I already have my citizenship certificate under my old name, do I still need to complete the whole application form? Surely the immigration office must have a copy of my data somewhere? Alternatively I could put an educated guess, say 1/1/1990 but I'll then be under the risk that they will reject my application because it doesn't match up with what they have on the system?

Also my father is not Canadian (nor any of my grandparents) - my understanding is that I still have to fill in section 8 with his details but with N/A for fields that are not applicable is that right?

Last thing - I have one additional citizenship since I obtained my Canadian one (so three citizenship in total) is that a problem?

Any advice on this would be much appreciated.

You need to find the correct dates by checking the old paperwork and if possible, please check with your mother.

Thanks for your reply. I've asked my my mum to check the paper works already but I don't think she has a copy - if I call up the consulate is this something they could help with?

Doesn't she have her old passport with the entry stamp and landing document which will have the entry date? Try finding it. The links that I have provided, please try to contact them to see what options you may have.
