One reason could be Spouse PCC not provided, or if provided, its not for complete period of time CIC asked for.

Yes, there have been couple of them primarily because of spouse PCC where it specifies only one address and he/she has stayed at multiple address before/after marriage during last 10 years.
Make sure PCC is geography and time independent for self/spouse.

Moreover, Make sure your provide documentary evidence for spouse education, language etc. if claiming points for same.
Not able to provide documents for anything you are claiming points for would lead to rejection

And if we are not claiming points for his work experience

Other than this is there any other readon for rejections ?

Do they cross verify spouses work experience ?

And spouse refference letter is needed ?

As mentioned above by fellow members, if you are claiming points and not attaching supporting documents to substantiate same, your application is deemed to be rejected. Anything else for which points are not claimed, no supporting documents are required.
