There is a post ITA form that you will be required to fill in which you can correct everything.

You will have to write a LOE explaining why name is different and then you will keep second guessing yourself if your application takes a long time, you will keep wondering whether it is due to this incident. Better to just get a fresh ITA and have mental peace if CRS score is well above the cutoff.
