regarding photos: How many numbers

I am new to this forum, ready to send my application. I am just confused whether, we need to paste one photo in the application apart from the 4 photos in the envelope.
I would appreciate, if anyone could calrify it.

Just in the envelope, one with the name and dob and date taken on the back.

As PMM said do not stick the pics.For all the people you are applying just write name & DOB on the back of one pic.Then put then in an envolope and write the name and DOB on the envelope as well to avoid any confusion.DO NOT PASTE ANY PICS.
