I am done with my medical test and I received a form with UMI number on it. I heard from the receptionist of the clinic that they have uploaded my results to CIC e-medical system.

Where can I find that URL?
How to track that the results have reached CIC successfully?

You will not have access to that report. You will just have to upload the form that you received as Proof of Medical Exam in your eAPR.

If you want the report and the date of delivery to CIC, you have to contact the clinic directly to issue a new eMedical form with those details, or simply ask them when they sent it.

Date of file upload, yes, if you ask the clinic they will let you know, but they do not share the report.

I checked with the receptionist this morning and they have uploaded the report a week back itseems. Also, she suggested me to use the UMI reference number to track my status. That's why I assumed there is a link for us to track it ;D

Hmm. I guess you mean the report that is sent to CIC, but I've been able to request the actual results from the clinic, including the disc and interpretation of the chest x-ray, for an additional fee.

I was informed otherwise by the clinic, may be the process might vary across geography.
