Regarding CAPIS....OMM, Qorax, PMM and others ....kindly help

Hi,

I am thinking to apply for capis note.

OMM I saw in one of your thread following steps to ask friend residing in canada to get CAPIS in $5.

But this link isn't working so I searched and found the new link http://www.cic.gc.ca/english/department/atip/form-imm5563.asp

Should I use form IMM5563, or 5475 or 5476? Please let me know, I think it shoudl be IMM5475, but want to confirm with you all here, before bothering my friend.

How to fetch the CAIPS thru friend/relative [in Canada] ?

Under the Privacy Act, one of our friends/relatives, who reside in Canada, can order the CAIPS under our authorization. There is a CIC fee of only $5 to be incurred for that purpose. Here's what we should do:

1. Download the form here: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563B.pdf
2. Fill the specified colums/fields, viz:
- a) Visa post [ND, Buffalo, London etc.]
- b) Your File number [Bxxxxxxx]
- c) Date of Birth [applicant's, not the rep's]
- d) The requester's Postal address [Canadian Address only]
3. Tick the box "for CAIPS Notes request" on the form.
4. Enclose copies of the rep's [not yours] PR Card/visa/COPR etc. [Canadian ID].
5. Attach the Consent Form [signed by each applicant for "use of a representative"].
6. Enclose a Money Order/Draft for $5.
7. Send (Mail or Courier 'only') all the documents to:
Citizenship and Immigration Canada
(Access to Information and Privacy Coordinator)

Ottawa, Ontario K1A 1L1
Tel.: (613) 957-6512
8. Your CAIPS Notes should return by 4-6 weeks [usually].

Hi!

I tell you how to order GCMS. As you have a relative / friend there it would cost you just $5. Its very easy. It can be done by email.

First fill this form, your (relative) requester will sign it.

then fill this consent form and sign yourself

Now you can even pay fees by your own credit card even if you are not living in Canada. Go to section C of the following form then follow other links given below.

After payment take the print out of receipt. Fill it, sign and scan.

Ask your relative to provide his Canadian passport /PR card and his recent bill / bank statement as a proof that he is currently living in Canada.

Tick both on form CAIPS and GCMS but they will send you GCMS only and it is much much better any way. Thanks

In this form, Section A (Client Information), although it clearly says Client info, just want to 100% sure, in Requester textbox (in Section A - Cleint Info) I have to write my Immigration File Number, is that right understanding?

section A. Put client details your name, date of birth , tick consent included , tick access to information act, tick privacy act, tick as is, tick in english, tick what format you want to receive by email, hard copy, or what

section B

tick both caips and gcms, write visa office like london, uk, write your file number,


in form IMM5475

no.4 write your UCI number and not file no. Its an 8 digit number called universal client identification. you can see that on your first or 2nd aor.
