Refusal letter ,reconsideration letter ,In process than to Decision made

Dear All (Please help) ,
I have applied under FSW on 25th April 2012 my e case shows initially Application recieved appx. after 2 months(July) it changes directly to Decision made in refusal letter IO states they are not satisfied by in Adaptability section the Blood relation points of my Real brother sa they are not satisfied with Residency proofs though as per direction checklist we have thoroughly submitted every doc.My agent has send them reconsideration letter on 27th july appx. and again their is automated mail came in which they wrote that already decision has been made in your case and that too has been intimated earlier .Now near 15th October my E case has been changed to IN PROCESS .After 2 weeks i.e on 1st november it again changes to Decision made n.ow i am totally confused and have foll . 2 questions.
1. What is my current status as i still in dilemma to send a reminder mail as my agent already twice send a letter last month for re review and a reminder.
2. What is meaning of IN PROCESS which was thier on E case for appx. 2 weeks(we have send reconsideration letter twice after refusal letter) and again changes to DECISION MADE on 1st NOV 2012.

Please guide what i need to do now .Is their any hope for Medicals and Visa as i am elogible for 5 points of My Real brother who is well settled from last 4 years in CANADA.

I have attached Passport ,PR card , Driving Licence ,Utility Bills , Bank Stateemnts ,tax returns ,Telephone Bills ,Gas electricity etc.
and pls let me know what id GCMS ...i ahven't applied for caps notes if it si same .
Please note i have send all these docs lus some additional in reconsideration letters and last month e case changes to IN process for 2 weeks and than again to DM on 1st nov.

I have attached my bro's Passport ,PR card , Driving Licence ,Utility Bills , Bank Statemnts ,tax returns ,Telephone Bills ,Gas electricity etc. as residency proof ...and relation proof they have accepted they are raising issue of residency proof in refusal letter ....that time ...though i have attached passport me and my brother ...School certificate and my Birth certificate all docs reflecting fathers name in same ...!

After refusal for a silly reason..... e case shown in " IN process" again ...and than to "decision made" again .... is this possible they work on reconsideration letters ?

is your brother working in Canada at the moment or elsewhere ? Like in USA under NAFTA etc ?

Sorry, I didn't see your request for assistance until just now.
Apply for GCMS notes under the Access to Information Act in Canada. You will find VO's details and an explanation of refusal in the notes. It would cost you only $5. Your brother can apply for the notes on your behalf as he is in Canada, but you have to supply him with a form giving your permission to get your notes. It can take more than 30 days to get them, so do not delay. Good Luck.

Shoot an email to CIC and ask about your status. In this way, you will come to know the real scenario of your case.

What supporting documents you attached to prove relation with your brother?
whats your visa office?
