References for passport application

Guys, so i am aware if we can not find a gurrantor we can sign a declaration form. Does anyone have any real experience with this, i mean do we just fill up the form and take it to the passport office or there is anything else to it as well?
Would really appreciate any guidance.

you can fill out the form PPTC 132. Check this:

I am aware of this, but my concern is with the last part "
The form must be sworn to or declared before, and signed by a person authorized by law to administer an oath or a solemn declaration"
What exactly this line refers to?

Notary. If you live around Toronto I can recommend a place I know that's generally cheaper ($10 vs $50 at Red Note for some stuff).

I apply from Germany and also need to fill out the PPTC 132. Can I just go to a German notary for this part or can this only be done at the embassy? Thank you!
