Reference letter format

Hello guys

I am preparing a reference letter for a previous experience where I held 2 titles

but there is a problem neither me nor the HR remember the transition date between the two titles but i am sure it's in september (we don't know the day exactly)

So should I mention only the month like this

Title 1
From 16/12/2017 to 9/2018

Title 2 from 9/2018 to 12/5/2019

Or can I randomly choose a day to mention ?

The key thing is if the two positions had different or same NoC. Since you were with the same company if noc didn't change you needn't worry about exact dates.
No change in NOC means no change in required duties. So it's basically the same job in the eyes of Immigration. Job title doesn't matter.
So because it's identical NOC in both positions you mentioned, you can use the same document for both, but just make sure the dates mentioned in your letter correspond exactly to the ones you entered in your profile. A simple discrepancy of 1-day can cause delays.

Yes then use two different dates with two titles and make sure those dates are matched same in your profile
