Both of my parents entered Canada last year, I applied for their extension and were granted for them to stay until end of January next year. They are planning to visit my sister in US for 6 months before their extension period end. Since my parents have multiple visa, can they reenter Canada after their 6 months stay in US without any problem? Or do they need to apply for another extension first before going to US? Please help!

They have a 10 year multiple visa. Their 1st stay was for 6 months but I applied for another 6 months extension and thats the one that will end in January next year.

No, their extension ends as soon as they leave Canada.

They may have issues if they have spent a year in Canada and then six months in the United States. CBSA may question if they're genuine visitors since that would be 1.5 years since they'd have been home. They may be refused entry. They may also be let in without any problems.

No, their extension ends as soon as they leave Canada.

but if there visa is still valid and have multiple entries advised them to re-enter using a flight i mean dont travel via road
if they re-enter using a flight and they have valid Canadian visa then it shouldn't be an issue

No matter how you enter (road, plane, boat, commercial camel), if you've spent a year in Canada and then six months in the US, the CBSA officers may see the US trip as a way to "reset the clock" and return to Canada again, meaning your parents are not genuine visitors.

Generally recommended that a visitor stays away from Canada for the same amount of time they were in Canada. This is why patents and grandparents should have a super visa to avoid this scenario and allows them to stay in the country for extended periods.

that is not to say they may arrive back from the US and be allowed to enter but they may find only authorised for a short period , weeks as opposed to another 6 months as CBSA may suspect they are living as visitors. No way to predict of course one way or other.
